HELP ASC Bio Form

This form allows you to fill in the necessary information for your bio page on HELP’s new website. Any field not marked as required is optional and will act as a survey for people’s willingness to include such information, for example, a child photo or quote.

We ask that you please use this form to submit your information so we can easily track your information and input it correctly on the new HELP website.

The information submitted by this form will be sent directly to HELP’s Knowledge to Action team and stored on our local server. By filling in and submitting this form you agree that any information you submit may appear on the HELP Website for public consumption.
